It is hard to maintain health-related records (lab tests and medical reports) safe and up-to-date, as they get torn, lost, or faded easily, making it useless for the doctors to understand history of the patients. Also, repeating the history for each doctor’s assistant is a time taking process and in case of emergencies, there may not be enough time to explain it to the doctors. Further, since the records are in no particular order, it is difficult for the health staff to establish correlation among different parameters which could have helped them in their prognosis. In our life cell phones are ubiquitous and are more reliable in carrying any information around easily. The idea is to develop an application that will be able to save and maintain medical records up-to-date. It will facilitate patients and doctors both by providing a reliable source of maintained history with analysis and predictions done through Artificial Intelligence, hence making the decision-making process easier.
For patients, our application enables them to keep an organized and managed collection of their medical records, access the general prescription based on their defined health conditions and share their medical records easily with their concerned doctors.
For doctors, our application enables them to access the patient’s record as well as to get a report of the analysis of their medical history and current health condition for better prognosis.
We are following component-based agile development model in which we have divided our work in three components which are database development, artificial intelligence and application development. Back-end development is done using PHP and React Native is used for application development.

Patients will be able to upload their past medical reports through OCR scanner.
Based on the medical history and the new conditions, there will be trends and predictions for the patient’s health making it easier for doctors to understand patient’s health.
The application will be able to propose general prescriptions, dosage, and time duration for its effectiveness based on the symptoms entered by the patient.
Patients’ records will be secured and will not be shared until and unless patient himself/herself wants to share it with someone.
If the patients want to share the record with concerned doctor, the patient will generate a request with the credentials or pass key.
The application will provide OCR functionality using Tesseract OCR to extract the parameters from patient's report and use of Artificial Intelligence for analyzing the report parameters. Generation of QR code for secure transfer of patients data to their respective doctors. Generation of prescriptions based on patient's input.
